  • • Your primary responsibility will be to coordinate and manage patient care pathways, ensuring that all clinical services, from initial contact to follow-up, are executed with precision and empathy. This involves liaising between patients, clinical staff, and external partners to streamline appointment scheduling, diagnostic test coordination, and the delivery of virtual care programs. You will be the central point of contact for many patients, providing them with clear information, support, and guidance throughout their care journey.
  • • A significant aspect of this role involves direct patient interaction, particularly with individuals who prefer to communicate in Spanish. You will conduct patient intake, gather essential medical history, explain treatment plans, and answer questions, all while ensuring cultural sensitivity and linguistic accuracy. Your ability to build rapport and trust with patients will be crucial in fostering positive patient experiences and improving adherence to care plans.
  • • You will meticulously manage patient records within our electronic health record (EHR) system, ensuring all documentation is accurate, up-to-date, and compliant with healthcare regulations. This includes scheduling diagnostic reads, coordinating televisits with our network of cardiologists and specialists, and tracking patient progress within our virtual care programs for conditions such as atrial fibrillation, vascular disease, and heart failure.
  • • Proactively identify and resolve any logistical or clinical barriers that may impede patient access to care. This could involve troubleshooting appointment conflicts, clarifying insurance information, or escalating complex patient needs to the appropriate internal teams or external resources. Your problem-solving skills will be essential in navigating the complexities of healthcare delivery.
  • • Collaborate closely with the clinical team, including physicians, nurses, and care managers, to ensure coordinated and comprehensive patient care. 🎯 Requirements
