📋 Description

• This role is a Bilingual (Spanish/English) Cardiometabolic Specialty Care Advanced Practice Provider at Devoted Health, Inc., focused on delivering exceptional preventive cardiovascular care to seniors with hypertension and atherosclerotic cardiovascular disease risk through a next-generation virtual specialty clinic.
• Day-to-day responsibilities include conducting virtual patient assessments, ordering and interpreting diagnostics, developing individualized treatment plans (including medication management and lifestyle modifications), initiating and managing statin therapy for high-risk patients, collaborating with an interdisciplinary team (physicians, APPs, pharmacists, social workers, health coaches), serving as a clinical advisor for escalation support, participating in panel reviews, utilizing and providing feedback on a home-grown EHR system, maintaining accurate medical records, and engaging in quality improvement and ongoing professional development.
• The role is part of Devoted Medical’s interdisciplinary virtual specialty clinic team, which operates as a “micro center of excellence” designed to expand access to specialized care for vulnerable seniors across 29 states, emphasizing compassionate, patient-centered, and technology-enabled care.
• The provider will deepen expertise in hypertension and ASCVD prevention, gain experience with innovative virtual care platforms and AI-assisted workflows, contribute to quality improvement initiatives, and grow professionally in a fast-paced, collaborative environment that values clinical excellence, empathy, and mission-driven innovation.

🎯 Requirements

• Active and clear RN and APRN or Physician Assistant license, with willingness to obtain and maintain multiple state licenses (Devoted operates in 29 states and covers licensing costs).
• 5+ years in outpatient clinical practice, including 2+ years focused on primary care or a relevant subspecialty (e.g., cardiology, nephrology, endocrinology) with heavy emphasis on hypertension and hyperlipidemia management.
• Strong clinical foundation in primary and secondary prevention of atherosclerotic cardiovascular disease (ASCVD), including evidence-based treatment strategies, medication management, and lifestyle modification.
• Ability to work Monday-Friday 8:30am-5:00pm MST or PST (EST and CST also considered).
• Fully bilingual in Spanish and English.
