  • • You will employ a diverse range of strategies, approaches, and techniques to actively support members in reaching their optimal wellness goals, with a strong emphasis on coordinating services and resources effectively.
  • • A key aspect of your role will be identifying and proactively resolving barriers that may hinder effective care delivery, ensuring members receive the support they need without unnecessary obstacles.
  • • Continuous monitoring of patient care is essential. You will achieve this through thorough assessments, diligent data review, meaningful conversations with members, and the active development and implementation of personalized care plans.
  • • You will be instrumental in facilitating the identification and referral process for D-SNP members, ensuring they gain timely access to the services that are most appropriate for their health conditions and needs.
  • • Promoting clear, consistent, and effective communication among all healthcare providers involved in a member's care is crucial for enhancing the overall quality of care delivery.
  • • Your role will involve coordinating services to prevent duplication of efforts and to guarantee that all of a member's health needs are comprehensively addressed, creating a seamless care experience.
  • • You will serve as a primary point of contact for external partners and agencies, maintaining open lines of communication and providing essential updates on member status and the services being rendered.
  • • The D-SNP Care Coach will collaborate closely with D-SNP coordinators to champion value-added benefits and support comprehensive member rounding initiatives, thereby ensuring a holistic and integrated case management approach for dual-eligible individuals.
  • • This is a field-based position requiring travel up to 25% of the time within the state of South Carolina, necessitating a valid driver's license and reliable personal vehicle.
  • • You will be responsible for maintaining accurate and detailed records of member interactions, care plans, and progress, utilizing various electronic information applications and software programs.
  • • A commitment to contributing to an organization dedicated to continuously improving consumer experiences is paramount, reflecting Humana's core values.
  • • This role is patient-facing and requires adherence to Humana's Tuberculosis (TB) screening program protocols.
  • • You will work from a dedicated, interruption-free space to ensure the privacy and security of Protected Health Information (PHI) and HIPAA compliance.
  • • While primarily remote, occasional travel to Humana's offices for training or meetings may be required, underscoring the need for flexibility and adaptability.

  • • The position involves a standard 40-hour work week, with compensation ranging from $53,700 to $72,600 annually, depending on experience and qualifications.
