  • • As a Care Coordinator at Health Advocate, you will provide essential support to Employee Assistance Program (EAP) members, ensuring they receive timely, compassionate care while navigating healthcare and behavioral health services. This role is vital in helping individuals access resources, manage care plans, and maintain well-being through coordinated follow-up and advocacy.
  • • Day to day, you will track and follow up with assigned members, contact them to confirm appointments, gather feedback on counseling services, research and identify in-network providers, triage incoming calls and chats, complete case management tasks, maintain accurate documentation in compliance with HIPAA, and escalate crisis situations to appropriate clinical resources.
