• • As a Care Coordinator on the Care Anywhere team, you will play a vital role in supporting seniors and chronically ill members by ensuring seamless coordination of home-based care services, directly contributing to improved health outcomes and reduced hospitalizations.
• • Your day-to-day responsibilities include managing provider schedules, preparing charts for home visits, conducting 60–80 outbound and inbound calls daily for scheduling, wellness checks, and care coordination, obtaining and uploading medical records to the EMR, submitting referrals for specialty services, DME, and home health, and assisting the NP team with visit preparation and documentation.
