- • As a Care Coordinator, you will make a lasting impact on members’ lives by ensuring their safety at home or within a community setting, traveling to members’ homes for visits while managing demands from internal and external stakeholders.
- • Day to day, you will partner with members and families to identify needed supports and direct services for health, employment, and independent living; collaborate with clinical and social support teams; conduct thorough face-to-face assessments covering physical, behavioral, functional, psycho-social, financial, and employment needs; and utilize criteria to authorize and verify home and community-based services.
- • You will join the Volunteer State Health Plan, Inc (VSHP), a subsidiary of BlueCross BlueShield of Tennessee, dedicated to improving health outcomes for diverse members in Davidson County and surrounding areas through compassionate, community-based care.
- • In this role, you will develop expertise in care coordination, clinical assessment, and community resource navigation while building strong interpersonal and organizational skills in a fast-paced, mission-driven environment focused on improving quality of life.
🎯 Requirements
- • 2 years of experience in a clinical setting
- • Registered nurse with an active license in Tennessee or a license in a participating Nurse Licensure Compact state; or Masters in Social Work with an active unrestricted license (LCSW, LMSW, or LAPSW)
- • Valid Driver’s License and ability to travel within Davidson County or within 30 miles
- • Exceptional customer service, time management, prioritization, and communication skills
- • PC skills (basic Microsoft Office and email) and successful completion of a digital literacy assessment
- • TB Skin Test (for field work) and willingness to comply with required immunizations and medical tests
