📋 Description
- • As a Care Manager - Behavioral Health at Humana Inc., you will play a vital role in supporting the health and well-being of members by assessing their needs, developing personalized care plans, and connecting them with essential resources—particularly those with behavioral health needs—through primarily telephonic interactions with occasional face-to-face support when required.
- • Your day-to-day responsibilities include conducting telephonic and in-person assessments, monitoring member progress toward health goals, creating individualized care plans, identifying and resolving barriers to care, collaborating with providers and community services, and working closely with an interdisciplinary team including community health workers, housing specialists, and SDOH coordinators to ensure holistic, person-centered support.
- • You will be part of Humana Healthy Horizons, a mission-driven initiative focused on removing barriers to health by partnering with state and local governments, community organizations, and national allies to improve access to care for Medicaid and Medicare members across Oklahoma and beyond.
- • In this role, you will deepen your clinical expertise in behavioral health care management, strengthen your ability to navigate complex social determinants of health, and grow your leadership in collaborative, team-based care environments—all while making a meaningful impact on vulnerable populations’ health outcomes.
