• • Day to day, you will evaluate member needs via phone or in-home visits, develop and monitor ongoing care plans for high-acuity individuals, coordinate care between members, families, providers, and community resources, and provide education on treatment options, resources, and benefits while ensuring compliance with state, federal, and payer regulations.
