📋 Description
- • The Care Manager, Registered Nurse (Remote) role at Sharecare, Inc. is critical to delivering high-quality telephonic care management that improves health outcomes, reduces costs, and enhances member experience within the Value Based Care Management program.
- • Day to day, the Care Manager will engage telephonically with members, caregivers, and providers to develop comprehensive care plans, assess ongoing needs, coordinate services, perform medication reconciliation, facilitate transitions of care, and maintain accurate documentation while advocating for members using motivational interviewing techniques.
- • Sharecare is a digital healthcare company that leverages data-driven AI insights and a comprehensive platform to improve care quality and drive personalized, value-based care across health plans, health systems, and pharmaceutical brands, serving members across the healthcare ecosystem.
- • In this role, the Care Manager will develop advanced clinical and interpersonal skills, gain expertise in care coordination and population health management, and contribute to measurable improvements in member health outcomes while working in a mission-driven, innovative healthcare technology environment.
🎯 Requirements
- • Current multi-state compact Registered Nurse licensure in state of residence is required, with ability to obtain additional licenses without restriction.
- • Minimum 3-5 years varied clinical experience with telephonic Case Management experience strongly preferred.
- • BSN preferred; training in motivational interviewing preferred.
- • Proficiency with Microsoft Office Suite (Word, Excel, Outlook, Teams) and ability to learn customized computer applications.
- • Ability to maintain a HIPAA-compliant home office with minimum internet connectivity requirements.
- • Demonstrated computer competencies including electronic medical records, word processing, spreadsheet, and presentation preparation.
