📋 Description

• Curate personalized provider lists for members seeking timely and appropriate healthcare services based on their specific care requests.
• Engage regularly with network providers and facilities to verify availability, credentials, and service offerings to ensure optimal care options.
• Collaborate with registered nurses (RNs) to navigate complex cases and effectively route members to the most suitable care settings.
• Confirm and update provider data in internal systems to maintain accuracy and compliance with access standards and service level agreements (SLAs).
• Document all member interactions and care coordination activities accurately in ticketing systems to ensure auditability and continuity of care.
• Identify and communicate emerging trends in member care requests and provider availability to leadership to drive process improvements and enhance member experience.
• Utilize all available internal tools and resources to resolve complex navigation challenges and support members through their healthcare journey.
• Stay current with all company policies, workflows, regulatory requirements, and updates to ensure consistent and compliant care navigation practices.
• Adhere to all applicable healthcare laws, regulations, and internal compliance standards in daily responsibilities.
• Perform other duties as assigned to support the goals of the Care Navigation Team and overall member experience objectives.

🎯 Requirements

• 1+ years of experience with care coordination and navigation in a healthcare setting
• 1+ years of experience utilizing a Chromebook and Google Workspace
