- ā¢ As a Care Support Partner at Served With Honor LLC, you will serve as the primary relationship holder and trusted point of contact for veterans and their families, providing compassionate, personalized guidance throughout their care journey.
- ā¢ Day to day, you will conduct intake assessments, develop individualized service plans, coordinate services using internal and community resources, manage client data in a secure portal, and collaborate cross-functionally to ensure seamless delivery of support.
- ā¢ You will work within a mission-driven organization dedicated to honoring veterans and their families by building authentic human and digital connections, ensuring no one navigates complex systems alone.
- ā¢ In this role, you will deepen your expertise in trauma-informed care, motivational interviewing, care coordination, and veteran-specific services while making a tangible impact on client outcomes and experience.
šÆ Requirements
- ā¢ Bachelorās degree in Social Work, Human Services, Healthcare Administration, or related field
- ā¢ 3-5 years of direct service experience in a person-centered healthcare or behavioral health setting
- ā¢ Experience in remote care coordination, digital navigation, and case management
- ā¢ Proficient in motivational interviewing, trauma-informed care, and culturally responsive approaches
- ā¢ Strong interpersonal and written communication skills for virtual engagement
- ā¢ Ability to build rapport, maintain trust, and follow through on commitments
