• • The Care Team Assistant role at Imagine Pediatrics PC is a remote position (EST required) that provides essential clinical and clerical support to ensure high-quality, compassionate, evidence-based care for children with special health care needs in a virtual Value-Based Care environment. This role matters because it enhances existing care teams by breaking down barriers to care and supporting families through proactive outreach, coordination, and documentation.
• • Day to day, the Care Team Assistant will accept and return patient phone calls involving clinical inquiries, perform protocol-based triage, route calls via EHRS phone notes, place calls and create template-based letters for lab results, review daily schedules to ensure pre-visit preparations are complete, contact patients at risk for no-shows or new patients for onboarding reminders, initiate forms for provider signature (e.g., disability forms, DME requests), and assist the care team in completing reports and identifying vulnerable patients for social service referrals.
