- ⢠As a Lead Care Manager at Vynca, you will serve as the primary point of contact for clients with complex health and social needs, ensuring seamless coordination across medical, behavioral, and community-based services to improve health outcomes and quality of life.
- ⢠You will play a pivotal role in advancing Vyncaâs mission to provide comprehensive care that enables more quality days at home by advocating for clients, integrating care across providers, and empowering individuals and families through trauma-informed, person-centered support.
- ⢠Your day-to-day responsibilities will include assessing member needs across physical health, mental health, substance use disorders, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and linkage to community-based services and supports.
- ⢠You will oversee the development of individualized care plans and SMART goal setting, ensuring they are client-driven, culturally responsive, and aligned with evidence-based practices such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles.
- ⢠You will deliver services where members reside, seek care, or find most accessibleâwhether office-based, telehealth, or field-basedâand actively connect clients to essential social services, including transportation arrangements (e.g., ACCESS) and referrals to housing, food security, and benefits programs.
- ⢠You will advocate on behalf of clients with healthcare professionals (e.g., PCPs, specialists, pharmacists), facilitate communication among caregivers, family support persons, and the broader care team, and promote wellness, recovery, independence, resilience, and member empowerment.
- ⢠You will conduct outreach and engagement activities to facilitate linkage to the Enhanced Care Management (ECM) program, log all interactions accurately in the Client Relationship Management (CRM) system, and evaluate client progress to update care plans and goals as needed.
- ⢠You will maintain up-to-date patient health records in the Electronic Medical Record (EMR) system and other business systems, complete all required documentation and outcome measures within established timeframes, and submit monthly reports to ensure program compliance.
- ⢠You will participate in assigned trainings to stay current on best practices, regulatory requirements, and Vyncaâs internal protocols, contributing to a culture of continuous learning and quality improvement.

đŻ Requirements
- ⢠2+ years of experience as a care manager, care navigator, or community health worker supporting vulnerable populations
- ⢠Willing and able to work MondayâFriday, 8:30amâ5:00pm Pacific Time, with flexibility for occasional evenings and weekends
- ⢠Working knowledge of government and community resources related to social determinants of health (e.g., housing, food insecurity, benefits programs)
- ⢠Clean driving record, valid driverâs license, and reliable transportation for field-based visits
- ⢠Excellent oral and written communication skills, with strong interpersonal abilities to engage diverse clients, families, and providers
- ⢠General computer proficiency, including working knowledge of Google Workspace, MS Office, and internet-based systems
- ⢠Bilingual (English/Spanish) preferred
