• • This role serves as the single point of contact for members to coordinate all care needs across service delivery systems and community supports, ensuring members receive appropriate, cost-effective, and least restrictive care.
• • Day-to-day responsibilities include conducting in-home and nursing facility visits, assessing member needs, developing individualized care plans, coordinating services across physical, behavioral, and long-term supports, facilitating care transitions, educating members and caregivers, collaborating with healthcare teams, documenting services in electronic health records, and ensuring compliance with HIPAA and state/federal regulations.
