📋 Description

  • • Day to day, the Case Manager will assess member health status, develop individualized care plans, coordinate services across providers, monitor progress, and advocate for members navigating the healthcare system. This includes conducting outreach, facilitating transitions of care, collaborating with interdisciplinary teams, and documenting all interactions in compliance with regulatory standards.

🎯 Requirements

  • • Active RN license in the state of Wisconsin or compact state licensure
  • • Minimum of 2 years of clinical nursing experience, preferably in case management, home health, or acute care settings
  • • Proficiency in electronic health record (EHR) systems and healthcare technology platforms
  • • Strong assessment, critical thinking, and communication skills with diverse populations
  • • Ability to work independently and manage a caseload in a remote environment
  • • Knowledge of Medicare, Medicaid, and commercial insurance guidelines
