- • Serve as the heartbeat of Ennoble Care’s mobile clinical teams, orchestrating seamless in-home visits for high-acuity patients across Philadelphia and surrounding counties. You will be the first point of contact, greeting patients in the comfort of their living rooms, setting the tone for compassionate, dignified care.
- • Own the full pre-visit workflow: confirm appointments via HIPAA-compliant telephony, verify insurance eligibility, reconcile medication lists, and prep electronic charts so clinicians walk in fully briefed and ready to heal.
- • Perform hands-on clinical support during house calls—vital signs, point-of-care testing, EKGs, wound care, phlebotomy, and specimen handling—while documenting findings in real time within our cloud-based EMR. Your accuracy and speed directly influence treatment decisions and patient safety.
- • Triage urgent patient and family questions between visits using secure messaging and telehealth platforms. You will decide when to escalate to an RN or physician, ensuring no symptom goes unaddressed and no family feels alone.
- • Coordinate multidisciplinary resources: schedule follow-up visits, arrange durable medical equipment deliveries, interface with pharmacies for same-day prescriptions, and loop in behavioral-health specialists or social workers when psychosocial red flags surface.
- • Champion Ennoble Care’s Remote Patient Monitoring (RPM) program by enrolling eligible patients, training them on Bluetooth-enabled devices, and reviewing daily data dashboards for anomalies. Your early detection of weight spikes in CHF patients or blood-sugar crashes in diabetics prevents hospitalizations.
- • Drive quality metrics that matter: maintain 95 % chart-closure within 24 hours, achieve patient-satisfaction scores above 4.8/5, and keep no-show rates under 5 % through empathetic, culturally sensitive communication.
- • Educate patients and caregivers on disease self-management, medication adherence, and advance-care planning. You will translate complex medical jargon into plain language so an 87-year-old with COPD or a newly diagnosed cancer patient feels empowered, not overwhelmed.
- • Participate in daily virtual huddles with physicians, NPs, social workers, and care coordinators to review high-risk cases, celebrate wins, and continuously refine workflows. Your frontline insights will shape clinical protocols and technology enhancements.
- • Uphold the highest standards of infection control and OSHA safety in non-traditional settings—whether you’re kneeling on a carpet to check pedal pulses or navigating narrow row-home staircases with portable equipment.
- • Contribute to grant-funded pilot programs and quality-improvement initiatives, collecting data that demonstrates how home-based care reduces ED visits and total cost of care. Your work will be cited in white papers and policy briefs that influence statewide healthcare reform.
- • Embrace a flexible, remote-first schedule that respects work-life harmony. While you’ll spend most days on the road, our AI-driven routing engine minimizes windshield time and maximizes face-to-face patient engagement.
🎯 Requirements
- • Active Certified Medical Assistant (CMA) credential from AAMA, AMT, NHA, or NCCT
- • Current BLS/CPR certification and reliable transportation with PA driver’s license and insurance
- • 1+ year of hands-on clinical experience in primary care, home health, hospice, or palliative settings
- • Proficiency with Epic, Cerner, or similar EMR; comfort toggling between mobile tablets and desktop dashboards
- • Bilingual in English and Spanish (or another prevalent Philadelphia community language) strongly preferred
