- ⢠Provides essential hands-on personal care and support for terminally ill patients in home hospice settings, directly contributing to patient comfort, dignity, and quality of life during end-of-life care as part of an interdisciplinary team.
- ⢠Performs activities of daily living including bathing, grooming, toileting, feeding, and mobility assistance for hospice patients in their homes, strictly following individualized care plans developed by registered nurses or therapists while maintaining accurate documentation of all care provided.
- ⢠Monitors and reports changes in patient condition, functional status, and emotional needs to supervising clinicians, ensuring timely interventions and effective communication across the care team to support holistic patient and family needs.
- ⢠Manages independent travel to patient residences across varying weather and traffic conditions, organizing daily schedules to meet visit requirements and deliver consistent, reliable care within allocated timeframes.
- ⢠Builds trusting, compassionate relationships with patients, family members, and caregivers, offering emotional support and guidance during difficult transitions while adhering to professional boundaries and ethical standards in home-based care.
- ⢠Collaborates with interdisciplinary team members including nurses, therapists, social workers, and chaplains to ensure coordinated, patient-centered hospice care that aligns with LVHNâs mission of excellence, compassion, and safety.

đŻ Requirements
- ⢠High School Diploma or GED equivalent
- ⢠Minimum of one year of experience as a nursing assistant or nursing aide in a healthcare setting
- ⢠Current Pennsylvania Certified Nursing Assistant (CNA) licensure or eligibility to obtain upon hire
- ⢠American Heart Association Basic Life Support (BLS) certification or ability to obtain upon hire
- ⢠Ability to drive reliably to patient homes in all weather and traffic conditions with a valid driverâs license and access to a vehicle
- ⢠Demonstrated ability to provide compassionate care and emotional support to terminally ill patients and their families
