📋 Description

  • Lead clinical integration efforts for newly acquired home health and hospice agencies, ensuring full alignment with organizational policies, regulatory standards, and quality benchmarks.
  • Develop and execute comprehensive transition plans for clinical operations, including staff orientation, competency validation, and targeted education programs for acquired clinical teams.
  • Collaborate with executive leadership and Portfolio Clinical Leaders to assess clinical infrastructure, identify operational gaps, and mitigate risks during acquisition and start-up phases.
  • Oversee end-to-end project management of clinical integration initiatives, including timeline development, task delegation, cross-functional stakeholder communication, and progress tracking to ensure on-time, compliant execution.
  • Identify opportunities for process improvement across clinical workflows, leading standardized initiatives that enhance efficiency, consistency, and quality of care across all acquired agencies.
  • Partner with Service Center Departments to streamline implementation of clinical systems, documentation protocols, and compliance procedures during transitions.
  • Provide direct guidance and support to agency leaders during post-acquisition stabilization, focusing on quality improvement, staff retention, and operational efficiency.
  • Monitor and report key performance indicators related to clinical outcomes, regulatory compliance, staff turnover, and adherence to accreditation standards throughout integration periods.
  • Serve as a subject matter expert on regulatory frameworks governing home health, hospice, and post-acute care, including state, federal, and accrediting body requirements.
  • Travel on-site to acquired agency locations to conduct assessments, facilitate training, and support operational transitions as required.
  • Utilize data-driven analysis to anticipate challenges, resolve complex operational issues, and implement sustainable solutions that align with organizational goals.
  • Maintain proficiency in Microsoft Office and web-based applications to manage documentation, reporting, and communication across decentralized teams.
  • Demonstrate exceptional verbal and written communication skills to influence stakeholders, train clinical staff, and present findings to executive leadership.
  • Manage multiple high-priority projects simultaneously with strong organizational skills, independently prioritizing tasks and meeting deadlines in a fast-paced, dynamic environment.
  • Promote and reinforce organizational culture, community values, clinical excellence, and financial accountability across all acquired entities.
