- • As a Clinical Case Manager at Catalight Foundation, you will play a pivotal role in enhancing the client family experience by focusing on process improvement, effective communication, client satisfaction, and the delivery of high-quality clinical outcomes.
- • Your primary objective will be to ensure client families receive comprehensive support throughout their care journey, proactively addressing any needs, barriers, or challenges they may encounter.
- • This role is central to providing exceptional service recovery by deeply understanding client-specific needs and clinical outcomes, thereby creating a more effective and supportive care path.
- • You will act as a crucial liaison, connecting client families, the provider network, internal Catalight departments (CCS), and external resources to assess and coordinate appropriate behavioral health treatment.
- • A key responsibility is to ensure that each client receives high-quality services through diligent care coordination.
- • You will meticulously follow established care plans, employing effective case management strategies to achieve desired client outcomes.
- • A critical aspect of the role involves assessing and addressing client risk, with a clear mandate to escalate high-risk clinical concerns to your supervisor promptly.
- • You will serve as the primary point of contact for client families and funders, ensuring all inquiries are addressed efficiently and effectively.
- • Conducting thorough clinical reviews of current care plans in collaboration with client families is essential for service recovery efforts, focusing on understanding barriers, challenges, clinical progress, and desired outcomes.
- • Your efforts will directly contribute to improving client satisfaction and access to medically necessary services by adeptly addressing and resolving any issues or complaints raised by client families.
- • You will be responsible for reviewing the clinical appropriateness of provider transfer requests, facilitating a smooth and supportive transition for clients to their next treating provider.
- • Accurate and timely documentation of all interactions with clients, providers, and the interdisciplinary team is paramount, adhering strictly to agency policies and utilizing appropriate systems.
- • You will provide essential support to Clinical Escalation Specialists and Provider Case Managers by conducting client case reviews and client family outreach to address complaints and grievances.
- • Documenting all client family complaints and grievances and submitting them to the Catalight Quality department is a required function, following established policy requirements.
- • You will support identified coverage needs related to Behavioral Health Screening, ensuring continuity of care.
- • Monitoring and analyzing trends in caregiver concerns will provide valuable insights for service improvement.
- • Preparing and presenting data on caregiver trends to management will inform strategic decision-making.
- • Active participation in special projects and assisting with additional duties or tasks as assigned by your supervisor will contribute to the broader goals of the foundation.
- • This role demands a strong understanding of community resources to effectively connect clients with necessary support systems.
- • Demonstrated expertise in crisis management is vital for effectively navigating challenging client situations.
- • You will be expected to conduct strengths-based, client-centered clinical screeners and refer clients for clinically appropriate services.
- • Adherence to all federal, state, and local laws/legislations, as well as HIPAA laws and regulatory agencies, is non-negotiable.
- • A high degree of sensitivity is required when working with ethnically, linguistically, culturally, and economically diverse populations.
- • You must demonstrate good judgment, flexibility, patience, and discretion when handling confidential and sensitive matters, aligning with the organization's values.
- • Proficiency in Microsoft Office Suite (Outlook, Word, Excel, etc.) is necessary for daily tasks.
- • The ability to thrive in a fast-paced environment while effectively multitasking is crucial for managing a demanding caseload.
- • You will collaborate effectively with various individuals across all organizational levels.
- • This role requires the ability to manage day-to-day responsibilities with or without direct supervision.

- • Visual and auditory ability to work with clients, staff, and others in the workplace continuously is necessary.
- • Frequent speaking and listening (25-75%) to clients, staff, and others in the workplace is expected.
- • Ability to utilize computer and cell phone (iPhone) is required.
- • Frequent sitting is a physical requirement of the role.
- • You must be able to obtain and maintain criminal record clearance through the Department of Justice (DOJ) and clearance through the Office of Inspector General.
- • Obtaining and maintaining a National Provider Identifier (NPI) is a condition of employment.
- • Attendance at any required training is mandatory.
