📋 Description

• As a Clinical MD at Hello Hera Inc., you will serve as the clinical entry point for older adults newly enrolled in our Medicare-covered community care management program, conducting comprehensive telehealth assessments that establish the foundation for personalized, ongoing care coordination and directly influence patient outcomes by identifying clinical priorities, synthesizing complex medical and social histories, and flagging risks such as medication confusion or fall hazards that often lead to preventable hospital readmissions.
• Your day-to-day responsibilities include conducting 45-60 minute telehealth visits with newly enrolled Medicare clients to confirm eligibility, perform in-depth clinical evaluations, document findings that guide care manager (Hero) interventions, collaborate with the care team to ensure alignment on care strategies, and provide clinical oversight as needed—all while building trust and rapport with patients and families during a vulnerable transition period.
• You will join a mission-driven, early-stage company dedicated to transforming geriatric care by replacing fragmented, volume-focused healthcare with proactive, relationship-based support in the community, where clinical leaders like you have autonomy to shape protocols, influence scaling into new states, and build systems that prioritize patient well-being over throughput.

🎯 Requirements

• MD or DO degree with active licensure to practice in one of the following states: Maryland (MD), Pennsylvania (PA), Massachusetts (MA), California (CA), Florida (FL), or New Jersey (NJ)
• Substantial clinical experience in geriatric medicine, primary care, or chronic disease management, with demonstrated ability to manage complex, multimorbid older adult patients
• Proficiency in conducting telehealth assessments and strong skills in building clinical rapport and trust with patients and families via virtual platforms
• Excellent clinical judgment, comfort navigating diagnostic and therapeutic ambiguity, and ability to prioritize care needs in socially complex situations
