📋 Description
- • As a founding clinical team member at Hello Hera Inc., you will play a pivotal role in shaping the future of geriatric care delivery by conducting comprehensive telehealth assessments that serve as the clinical foundation for ongoing, Medicare-covered support for older adults across multiple states. Your work directly enables families to receive personalized, continuous care management grounded in clinical excellence and compassionate engagement.
- • You will conduct 45–60 minute telehealth visits with older adult patients, synthesizing complex medical histories, social determinants of health, and functional status to confirm Medicare eligibility, identify care barriers, and establish trusting relationships that empower families to actively participate in care planning.
- • Each assessment you perform will inform the entire care strategy for your assigned clients, guiding coordination with caregivers, specialists, and community resources to ensure holistic, person-centered support over time.
- • You will exercise strong clinical judgment in ambiguous or complex cases, leveraging your expertise in geriatric syndromes, polypharmacy, and chronic disease management to make safe, evidence-based recommendations that improve quality of life and reduce avoidable hospitalizations.
- • You will collaborate as part of a growing, mission-driven clinical team, contributing insights to refine assessment protocols, care pathways, and team-based workflows as Hello Hera expands into new states and scales its model of community-based geriatric care.
- • You will enjoy significant autonomy in structuring your weekly schedule (3–10+ hours per week), allowing you to balance clinical impact with personal flexibility while maintaining a meaningful caseload of 40+ clients.
- • You will develop deep proficiency in virtual geriatric assessment techniques, honing your ability to build rapport, detect subtle clinical cues, and navigate sensitive conversations about aging, independence, and end-of-life preferences—all through a telehealth platform.
- • You will gain firsthand experience in innovative care delivery models that integrate clinical expertise with technology-enabled care management, positioning you at the forefront of value-based, aging-in-place healthcare innovation.
🎯 Requirements
- • Licensed Nurse Practitioner (NP), Physician Assistant (PA), or Medical Doctor (MD) with active, unrestricted licensure to practice in at least one of the following states: New Jersey (NJ), Maryland (MD), Pennsylvania (PA), Massachusetts (MA), California (CA), or Florida (FL)
- • Substantial clinical experience in geriatric primary care, transitions of care, or chronic disease management, preferably in outpatient, home-based, or value-care settings
- • Proven ability to conduct effective telehealth assessments, including establishing trust and clinical rapport with older adults and their families through virtual interactions
- • Strong clinical judgment, excellent verbal and written communication skills, and comfort managing clinical complexity, diagnostic uncertainty, and psychosocial ambiguity
