- • The Clinical Program Consultant, Virtual Care Management (VCM) serves as the organization’s subject matter expert in virtual care program design, implementation, and optimization, playing a critical role in ensuring program excellence, scalability, compliance, and measurable outcomes for key programs including Principal Care Management (PCM), Chronic Care Management (CCM), Transitional Care Management (TCM), and Remote Patient Monitoring (RPM).
- • Day to day, the consultant leads the design, refinement, and scaling of virtual care management programs; establishes and standardizes best practices, workflows, and operating procedures; ensures alignment with regulatory requirements and payer guidelines; defines KPIs and success metrics; analyzes program data to drive performance improvements; provides guidance on program expansion and payer strategies; and trains and mentors VCM management and frontline staff on protocols and best practices.
- • The role is part of Cardiac RMS by DocGo, an innovator in digital health and remote care services, which is a Medicare-approved Independent Diagnostic Testing Facility (IDTF) providing Cardiac Implanted Electronic Device (CIED) monitoring and Virtual Care Management Services using a proven cloud-based platform that connects patients, providers, and healthcare teams.
- • In this role, the individual can advance best practices and performance measurement strategies across virtual care programs, drive continuous improvement, influence strategic decisions through data analysis, mentor teams, and develop expertise in compliance, billing, and value-based care models within a growing digital health organization.
🎯 Requirements
- • Significant experience in virtual care management programs (PCM, CCM, TCM, RPM)
- • Demonstrated success in program development, implementation, and optimization
- • Strong understanding of CMS guidelines, billing requirements, and compliance considerations
- • Experience developing workflows, SOPs, and performance metrics
- • Proven ability to analyze data and drive operational improvements
- • Experience training, mentoring, or advising care management teams
