📋 Description
- • Serve as a liaison between health and social services and the community to address members’ housing concerns and connect them with community resources.
- • Assist members with completing housing applications, identifying available housing vacancies, and adding them to waitlists for housing programs.
- • Educate members on understanding and utilizing their housing benefits, particularly those related to Medicaid eligibility and policies.
- • Conduct in-person assessments to evaluate members’ care needs, socioeconomic barriers, and home environments to inform care planning.
- • Navigate members through healthcare and social service systems, coordinating access to basic needs including housing, food, income support, and transportation.
- • Identify and remove barriers to housing that prevent members from attending medical appointments or accessing care.
- • Provide consistent, non-judgmental social support to improve members’ morale, self-worth, and sense of stability.
- • Conduct research and in-person outreach to locate difficult-to-contact members and increase participation in clinical and support programs.
- • Build and maintain relationships with community providers and organizations to facilitate referrals and identify gaps in local resources.
- • Travel up to 25% to member homes, facilities, and community locations to conduct visits and ensure effective program delivery.
- • Work collaboratively with care management teams to support case management functions and address social determinants of health.
- • Maintain a dedicated, private home office with a locked door to ensure confidentiality and compliance with HIPAA regulations for member PHI.
- • Utilize Microsoft Office applications (Teams, Excel, PowerPoint, Outlook, Word) for documentation, reporting, and communication.
- • Engage with Medicaid enrollees, including those with physical and behavioral health needs and varying levels of health literacy.
- • Participate in remote interviews via HireVue, responding to recorded or text-based questions as part of the hiring process.
- • Adhere to Humana’s work-at-home internet requirements: minimum 25 Mbps download and 10 Mbps upload speed via wired cable, DSL, or approved wireless connection.
- • Maintain a valid state driver’s license and personal vehicle liability insurance meeting state minimums or $25,000 bodily injury per person/$25,000 per event/$10,000 property damage.
- • Eligible for mileage reimbursement for work-related travel, including trips from home to first location, between client sites, and back home.
- • Complete TB screening as required due to patient-facing responsibilities.
🎯 Requirements
- • Must reside in the state of South Carolina
- • Able to travel up to 25%
- • Minimum two (2) years of experience with community resources, health agencies, or social services (e.g., Area Agency on Aging, DME providers, Meals on Wheels)
- • Proficiency in Microsoft Office (Teams, Excel, PowerPoint, Outlook, Word)
- • Must have a separate room with a locked door that can be used as a home office to ensure continuous privacy while working
- • Strong written and verbal communication skills and ability to advocate for members at all care levels
