📋 Description

  • Supervise Clinical Managers, Clinical Consultants, Lead Care Managers, and support staff to ensure effective delivery of the Enhanced Care Management (ECM) program
  • Refine and implement program policies and procedures aligned with contractual and regulatory requirements
  • Oversee day-to-day operations of the ECM program to ensure consistent service delivery and compliance
  • Ensure all ECM contract goals, objectives, SLAs, and performance metrics are met and exceeded
  • Maintain compliance with all regulatory and data reporting requirements for ECM programs, including CalAIM
  • Ensure electronic and physical records, billing practices, and documentation standards meet organizational and legal requirements
  • Oversee assessments, intakes, care planning, crisis management, and transition plans for patients with complex needs
  • Direct and ensure comprehensive staff training programs are developed and executed
  • Generate and analyze EHR reports to monitor program performance and inform decision-making
  • Oversee claims submission, denial management, and payment receipt processes to ensure financial accuracy and timeliness
  • Represent the organization in meetings with health plans, regulatory bodies, vendors, local businesses, and community stakeholders
  • Prepare and deliver reports to management and managed care plans regarding ECM program outcomes and metrics
  • Manage CalAIM program performance and ensure successful execution of all service level agreements
  • Design and implement standard operating procedures and workflows to optimize department efficiency
  • Develop strategic messaging to communicate the company’s value proposition and address patient objections
  • Manage quality assurance programs to enhance client experience and care outcomes
  • Establish and monitor KPIs that align team performance with business objectives
  • Manage P&L for the ECM program and present accurate financial results during monthly operations reviews
  • Apply analytical and creative problem-solving to address business challenges and adapt quickly to changing priorities
  • Collaborate with business development and clinical operations teams to streamline referral management and patient onboarding
  • Model and reinforce the company’s core values of Excellence, Compassion, Curiosity, and Integrity across all team interactions
  • Perform other related duties as assigned

🎯 Requirements

  • Bachelor’s Degree, highly preferred
  • 5+ years of people management experience
  • 3+ years of care management experience
  • Experience with Enhanced Care Management programs, highly preferred
  • Proven experience with P&L responsibility
  • Proficiency with Microsoft and Google applications and general computer literacy
