📋 Description

• The ECM Clinical Manager (RN) provides remote clinical oversight and management of Enhanced Care Management staff to address clients' medical, behavioral health, and social determinants of health needs, playing a critical role in delivering comprehensive care for more quality days at home.
• Day-to-day responsibilities include supervising a team of 10-15 direct reports, overseeing client outreach and enrollments, driving KPI achievement, providing clinical oversight 30% of the time, leading Interdisciplinary Care Team meetings, and offering direct care management as needed.

🎯 Requirements

• Active, unrestricted RN license with ability to obtain California licensure within 6 months of hire
• 2 years of people management experience (4 years preferred)
• 2-3 years of care management experience
• 1-2 years of post-acute experience
• Proven results-driven leadership with change management experience in rapid-growth environments
• Proficient in Microsoft and Google applications; Salesforce experience is a plus
