  • • Continuous monitoring of member progress is essential. Through regular assessments and evaluations, you will ensure that members are advancing toward their individualized care goals and that care plans remain relevant and effective.
  • • You will be responsible for developing and implementing personalized care plans. These plans will be based on your clinical judgment, informed by changes in member health status, psychosocial factors, and identified triggers, ensuring targeted and effective clinical interventions are provided.
  • • Effective collaboration is key to success in this role. You will work closely with healthcare providers, community organizations, and various service providers to facilitate the delivery of high-quality, cost-effective care and support for members, ensuring a coordinated approach to their health journey.
  • • You will coordinate the delivery of essential services, addressing not only physical health needs but also the critical social determinants of health and other value-added benefits that comprehensively support members in their entirety.
  • • Facilitating seamless communication and coordination across the interdisciplinary care team, including primary care providers, is paramount to ensuring integrated service delivery and smooth transitions of care for members.
  • • A strong understanding of departmental, segment, and organizational strategies and objectives is required, ensuring your daily practices align with broader operational goals and contribute to Humana's mission.
  • • Accurate and timely documentation is a critical component of this role. You will be responsible for meticulously documenting all assessments, interventions, and progress, as well as accurately submitting incident reports in accordance with organizational policies and regulatory requirements, ensuring compliance and quality assurance.
  • • This role requires a commitment to patient safety and well-being, including participation in Humana's Tuberculosis (TB) screening program. You will also be expected to maintain a safe driving record and ensure personal vehicle liability insurance meets or exceeds state minimums, as travel is a significant component of the role.
  • • You will leverage intermediate to advanced computer skills, including proficiency with Microsoft Word, Outlook, and Excel, and possess excellent keyboard and web navigation skills for efficient documentation and information retrieval.
  • • The ability to work with minimal supervision, demonstrating initiative and sound judgment within the scope of your role, is essential for success in this field-based position.
  • • You will be expected to reside in Louisiana and maintain a flexible work schedule to accommodate member needs, including potential overtime as required, while traveling within a 50-mile radius of your residence for home visits.
