📋 Description

• The Health Home Clinician role at Centerstone is vital for delivering integrated behavioral and physical health support to clients in their natural environments, directly contributing to personal growth, wellness, and improved life outcomes through evidence-based, person-centered care.
• Day to day, the clinician will provide individual and group therapeutic interventions, assist clients in developing Wellness Recovery Action Plans (WRAP), conduct assertive outreach and follow-up, manage cases including assessment and service coordination, support clients with physical health needs through collaboration with primary and specialty care providers, offer transportation assistance, build natural support networks, and serve as representative payee when clinically indicated, all while documenting services and participating in team meetings and quality assurance activities.

🎯 Requirements

• Bachelor’s degree required, or equivalent qualifications as a Mental Health Professional with five years of paid experience in the mental health field and a high school diploma, subject to approval.
• Valid driver’s license and safe driving record required for providing transportation assistance and conducting community-based outreach.
• Demonstrated ability to communicate effectively via written, verbal, in-person, and virtual methods with clients, families, and interdisciplinary teams.
• Skill in customer service/experience techniques and problem-solving to support client engagement and goal attainment.
• Knowledge of community resources and their roles in supporting clients across life domains such as housing, employment, and healthcare.
