- • The Health Navigator role at BlueCross BlueShield of Tennessee supports members in understanding discharge plans, scheduling follow-up appointments, managing medication changes, and addressing home-based needs as part of the SeniorCare team.
- • Day-to-day responsibilities include conducting educational telephone calls, reaching out to members with care gaps, managing work queues, facilitating research on member inquiries, and working overtime as needed.

🎯 Requirements
- • Associates Degree in education, communication, or health-related field or equivalent work experience
- • 2 years of experience in a customer service support role
- • Knowledge of medical terminology
- • Ability to work independently with minimal supervision or in a team environment
- • Strong interpersonal, organizational, and communication skills
- • Self-motivated with effective time management and problem-solving abilities
