📋 Description
- • Serve as the primary external representative of A Place for Mom to hospitals, skilled nursing facilities, and discharge planners within your assigned territory, building and maintaining trusted relationships with social workers, case managers, and discharge planners
- • Drive lead generation and move-ins by consistently generating qualified patient referrals each day from your portfolio of healthcare accounts, ensuring alignment with monthly targets for family placements in senior living communities or home care services
- • Develop, own, and expand your book of accounts by initiating new outreach through cold calls, in-person visits, and scheduled service presentations to healthcare professionals at hospitals and skilled nursing centers
- • Follow up systematically with referral sources on discharge outcomes to reinforce the value A Place for Mom delivers and strengthen long-term partnership reliability
- • Educate families transitioning from hospitals or skilled nursing facilities about senior care options, clarifying how they will collaborate with Healthcare Senior Living Advisors to identify the most appropriate community or home care solution
- • Partner closely with Healthcare Senior Living Advisors to ensure seamless coordination between referral sources and families, jointly achieving monthly placement quotas
- • Leverage CRM data and internal reporting tools to track, manage, and optimize your sales pipeline, identifying trends and adjusting outreach strategies for improved conversion rates
- • Pilot new initiatives such as account scoring models, CRM enhancements, or process improvements within your territory, providing actionable feedback to refine company tools and resources
- • Travel daily within your local territory during the 5-day work week to conduct in-person meetings with healthcare professionals, attend discharge briefings, and make impromptu visits to facilities
- • Maintain high standards of time management and organizational discipline to balance field activities, administrative tasks, and follow-up communications while consistently meeting performance expectations
- • Utilize Microsoft Office, Google Sheets, and Salesforce (preferred) to document interactions, track metrics, and ensure accurate reporting across all sales activities
- • Remain adaptable in a fast-paced, evolving environment, embracing change and proactively testing new approaches to increase referral volume and service impact
- • Demonstrate persistent, relationship-driven sales behavior with a strategic mindset, overcoming objections and closing opportunities through empathy, professionalism, and persistence
- • Contribute to a mission-driven culture focused on helping families find care that meets their needs while upholding company values of integrity, collaboration, and continuous improvement
🎯 Requirements
- • 3-5 years of outside sales experience as an individual contributor with proven prospecting and lead generation abilities
- • Hospital or skilled nursing facility sales experience
- • Proven track record of exceeding sales quotas and collaborating cross-functionally to achieve outcomes
- • Proficiency in CRM systems (Salesforce preferred), Microsoft Office, and Google Sheets
- • Bachelor’s degree preferred
- • Must be willing to travel daily within territory during the 5-day work week
