- • Serve as the primary external representative of A Place for Mom to hospitals and skilled nursing facilities within your assigned New York City territory, building and maintaining relationships with social workers, case managers, and discharge planners.
- • Generate daily qualified patient referrals from assigned accounts to meet and exceed monthly targets for families selecting senior care options, including community placements or home care services.
- • Develop, own, and expand your book of accounts by identifying new referral sources, conducting cold calls, and delivering service presentations at hospitals and skilled nursing facilities.
- • Follow up with referral sources on discharge outcomes to reinforce the value A Place for Mom delivers and strengthen long-term partnerships.
- • Educate families of seniors being discharged from hospitals or skilled nursing facilities on senior living options and coordinate with Healthcare Senior Living Advisors to guide families toward the most appropriate care solution.
- • Collaborate closely with Healthcare Senior Living Advisors to ensure coordinated efforts in driving move-ins and home care placements aligned with your referral pipeline.
- • Use CRM reports and internal data tools to build, manage, and optimize your sales pipeline, tracking referrals, conversions, and account performance metrics.
- • Pilot and test new initiatives such as account scoring models, CRM enhancements, or outreach processes within your territory and provide actionable feedback to improve company-wide tools and resources.
- • Travel daily within your New York City territory during the standard five-day work week to conduct in-person visits with healthcare professionals, attend discharge meetings, and make impromptu visits to facilities.
- • Maintain consistent communication with internal stakeholders including your Regional Director to build weekly account plans, align on priorities, and report on progress toward monthly quotas.
- • Demonstrate strong objection-handling skills during presentations to healthcare professionals and families, effectively communicating the benefits of A Place for Mom’s services.
- • Maintain proficiency in Microsoft Office, Google Sheets, and Salesforce CRM to manage contacts, track interactions, and generate reports essential to sales performance.
- • Operate independently in a fast-paced, ever-evolving environment with a high degree of autonomy while remaining highly responsive to changing priorities and new initiatives.
- • Contribute to company goals by embracing change, testing innovative approaches, and consistently seeking ways to improve processes and outcomes in your territory.
