📋 Description
- • The Hospice RN Inpatient Liaison – Weekends role is critical to ensuring seamless, compassionate care transitions for hospice patients moving between inpatient and home settings, directly supporting ThedaCare’s mission to reinvent healthcare through proactive, patient-centered partnerships.
- • Day-to-day responsibilities include assessing patient conditions using the nursing process, developing individualized care plans in collaboration with interdisciplinary teams, managing acute and chronic symptoms, coordinating discharges, and providing expert clinical guidance to patients, families, and care teams during weekend shifts.

🎯 Requirements
- • Current Wisconsin RN Licensure
- • American Heart Association Healthcare Provider Basic Life Support (BLS) certification
- • BSN completion required within five years for non-BSN nurses with less than one year of RN experience upon hire
