📋 Description
- • The Hospital-Community RN Liaison - Home Health role at Mercy Health is a dynamic position responsible for accepting Hospice, Home Health, and Home Infusion referrals across acute care, physician clinics, nursing homes, and rehabilitation centers. This role ensures timely discharges and supports continuity of care within the Mercy Health System by maintaining an active presence in clinical settings and delivering excellent customer service to discharge planners, medical social workers, physicians, and future patients.
- • Day-to-day responsibilities include evaluating referrals, coordinating with healthcare teams to facilitate smooth transitions from hospital to home-based care, assessing patient eligibility for hospice and home services, communicating service options to physicians and post-acute providers, documenting assessments and interactions accurately, and serving as a trusted liaison between acute care settings and community-based care teams.

🎯 Requirements
- • Current RN license in the applicable state of practice
- • Minimum three years of nursing experience
- • Bachelor’s degree in Nursing (BSN) preferred
- • Experience in home health, hospice, or acute inpatient settings preferred
- • Strong observation, nursing assessment, communication, and documentation skills
- • Ability to work independently with minimal supervision and collaborate effectively within a team
