📋 Description
- • Vynca is seeking a highly motivated and detail-oriented Intake Specialist to join our growing team and play a pivotal role in transforming care for individuals with complex needs. This remote position, based in the United States, offers a unique opportunity to contribute to our mission of providing comprehensive care for more quality days at home.
- • As an Intake Specialist, you will be instrumental in facilitating the patient enrollment process, working closely with our Sales/Marketing and Outreach Specialists teams. Your primary responsibility will be to ensure a smooth and efficient intake process, establishing positive relationships with both internal teams and the patients and families we serve.
- • This role is critical to our operations, and we are looking to fill it as soon as possible. You will be the first point of contact for many patients, setting the tone for their experience with Vynca and ensuring they feel supported and understood from the outset.
- • Key responsibilities include facilitating the intake and admission of incoming patients. This involves meticulously managing all documentation related to the admissions process, ensuring accuracy, completeness, and compliance with all Vynca protocols and regulatory requirements.
- • You will conduct accurate and ongoing assessments of patient status during the admission phase. This requires a keen eye for detail and the ability to gather and interpret essential information to ensure patients are appropriately placed and receive the care they need.
- • A significant part of your role will involve providing essential administrative computer support. This includes maintaining various databases, logs, and census sheets, ensuring all patient information is up-to-date and easily accessible. You will also be responsible for updating and maintaining resource/contact lists, schedules, and other department-specific information sheets, which are vital for seamless team collaboration and patient care coordination.
- • Scheduling and completing pre-admission assessments is another core function. You will communicate recommendations clearly and compassionately to patients and their families, ensuring they understand the next steps and have all their questions answered.
- • The Intake Specialist will actively participate in all staff, census, and other relevant meetings. This includes attending weekly meetings with the Outreach Team and other stakeholders, contributing insights, and collaborating on strategies to improve the intake process and patient outcomes.
- • A crucial aspect of this role is meeting or exceeding established admission targets. Your efficiency and effectiveness in managing the intake pipeline will directly contribute to Vynca's growth and ability to serve more individuals.
- • You will also be responsible for answering incoming calls, providing a high level of customer service, and directing inquiries appropriately. This requires excellent communication skills and the ability to handle a high volume of calls professionally and empathetically.
- • In addition to these core duties, you will be expected to assume other responsibilities in support of organizational needs, demonstrating flexibility and a proactive approach to contributing to Vynca's overall success. This might involve assisting with scheduling if needed, or other tasks that arise in a dynamic healthcare environment.
- • The ideal candidate possesses strong organizational and customer service skills, with prior experience in healthcare settings being highly advantageous. Effective communication, technological proficiency, and robust relationship-building skills are essential for success in this role. You will be a key liaison, bridging the gap between potential patients and the Vynca care team, ensuring a compassionate and efficient transition into our programs.

🎯 Requirements
- • Minimum of 2 years of previous intake or admission experience in a high call volume environment.
- • 2+ years of experience in palliative care, home health, hospice, and/or hospital discharge planning or case management, highly preferred.
- • Strong understanding of cultural competency with the target population.
- • General computer skills, including a working knowledge of the internet and the ability to independently learn and use Google Suite and Salesforce.
- • Willing and able to work Monday-Friday 8:30 am - 5:00 pm Pacific Time, with the possibility of evenings and weekends.
- • Bilingual (English/Spanish) proficiency, highly preferred.
