- • As the Medical Director of Clinical Programs and Innovation at Chamber Cardio, you will play a pivotal role in shaping the future of cardiovascular care. This is a unique opportunity to lead and scale our innovative clinical programs, including critical initiatives like Transitions of Care (ToC), Guideline-Directed Medical Therapy (GDMT), and the development of new condition-based interventions. You will be instrumental in guiding our dedicated care team towards clinical excellence, collaborating closely with our Product team to design and implement cutting-edge, tech-enabled workflows, and driving the creation of novel care models that demonstrably improve patient outcomes across the populations we serve.
- • Your leadership will extend to overseeing the day-to-day operations and strategic direction of Chamber’s core clinical programs. This includes ensuring strict adherence to the latest evidence-based protocols, fostering a culture of continuous improvement, and championing best practices in patient care. You will be responsible for the direct supervision and mentorship of our talented Nurse Practitioners and Care Coordinators, cultivating an environment where patient-centered, protocol-driven care thrives. A significant aspect of your role will involve the development and piloting of new clinical programs, including exploring and establishing emerging pathways that address unmet needs in cardiovascular health.
- • A key facet of this position is ensuring seamless and effective collaboration with our network of independent cardiologists and other healthcare providers. You will work to align on unified standards of care, shared population health goals, and integrated care delivery strategies, ensuring that Chamber Cardio acts as a true partner in advancing community heart health.
- • In the realm of clinical innovation and product collaboration, you will be a crucial bridge between clinical needs and technological solutions. You will partner extensively with our Product, Engineering, and Analytics teams to conceptualize, design, and rigorously test AI-enabled clinical tools, sophisticated workflows, and insightful dashboards. Your ability to translate complex clinical requirements into clear, actionable product specifications will be vital in streamlining care delivery, enhancing patient monitoring, and improving the accuracy of our outcome measurements.
- • You will actively lead clinical input into our product roadmaps, influencing the development of new features that aim to significantly enhance both the patient and provider experience. Furthermore, you will be responsible for evaluating and operationalizing innovative solutions that drive clinical efficiency, boost patient engagement, and ultimately lead to better health outcomes. This includes staying abreast of emerging technologies and methodologies in digital health and cardiology.
- • Data-driven outcomes and quality assurance are at the heart of Chamber Cardio’s mission. You will define, meticulously monitor, and analyze key performance metrics related to patient outcomes, healthcare utilization, and medication adherence across all our programs. Your insights will be critical in leading outcomes reviews and engaging in productive discussions with payers, focusing on demonstrating our performance and the tangible impact of our interventions.
- • You will collaborate closely with our analytics team to proactively identify opportunities for targeted interventions, process improvements, and the strategic automation of clinical tasks. Establishing robust feedback loops between our care teams, technology platforms, and senior leadership is essential for driving a cycle of continuous quality improvement and innovation. This ensures that our clinical strategies are informed by real-world data and frontline experiences.
- • As a cross-functional leader, you will work seamlessly with our Population Health, Operations, and Network teams to translate high-level strategy into effective, scalable execution. You will serve as the primary clinical voice in meetings with payers, providers, and strategic partners, articulating the unique value proposition of Chamber Cardio’s model. Your contributions to internal leadership discussions on care design, quality improvement initiatives, and program expansion will be invaluable as we grow and evolve.
- • In your first 90 days, you will focus on establishing consistent, high-quality outcomes for NP-led care delivery within our GDMT and ToC programs. You will lead the initial performance and quality reviews of key metrics for outcomes and patient engagement. A critical task will be supporting the onboarding and comprehensive clinical training of new care team members, ensuring adherence to our quality standards and operational protocols. You will also collaborate closely with the Product team to facilitate the successful launch of Chamber’s AI-enabled clinical workflows and provider tools, marking a significant step in our technological advancement.
