- • As a Member Care Navigator at Better Life Partners Inc., you will serve as a primary point of contact for members navigating addiction recovery, helping them understand, coordinate, and access essential care and support services. This role is vital to ensuring members feel seen, heard, and cared for throughout their recovery journey.
- • Day to day, you will engage with members via virtual and in-person channels to assess needs, provide compassionate guidance, assist with scheduling counseling and medication access, and connect individuals with community-based resources. You will document interactions accurately, follow up on care plans, and maintain consistent, responsive communication to support continuity of care.
- • Better Life Partners is a mission-driven organization dedicated to healing from addiction by meeting people where they are. We partner with community-based organizations to deliver integrated support through counseling, medication access, and community engagement, fostering belonging, love, and purpose in recovery.
