📋 Description

• Lead and scale interdisciplinary care teams nationally to support patients across the full acute care continuum, fostering a culture of clinical excellence and rigor.
• Serve as the chief clinical architect for Devoted Health’s next-generation episodic care model in the post-ED, post-hospital, and post-acute care space, defining a proprietary blend of human expertise and AI-driven precision.
• Drive strategy and business success for episodic care programs, ensuring durable patient outcomes beyond the critical 7–30 day window through targeted interventions in high-value metrics: Plan All-Cause Readmissions (PCR), Follow-up in patients with multiple co-morbidities after ED visits (FMC), and Transitions of Care (TRC).
• Instrument and optimize an AI-enabled system that surfaces personalized, high-value care pathways tailored to individual patient contexts and clinical needs.
• Scale a high-touch, virtual in-home "observation" level of care program to serve patients at high risk of rehospitalization due to new or unresolved acute conditions.
• Act as the Medical Leader for the Patient Safety Organization (PSO), leveraging acute event intelligence to implement enterprise-wide safety patterns and quality standards.
• Hire, manage, and develop leaders of a growing team of physicians and advanced practice providers across a distributed national workforce.
• Design and implement performance management systems that balance clinical outcomes, patient experience, staff engagement, and productivity at scale.
• Collaborate closely with engineering, product, and data teams to shape technology-enabled care delivery—no coding required, but must think critically about how technology transforms clinical workflows.
• Maintain active clinical practice in a virtual care setting as a board-certified physician to remain grounded in frontline patient care.
• Champion the adoption of AI and technology as tools to enhance, not replace, the craft of medicine, with a bias toward rapid iteration, learning, and adaptation.
• Build new clinical service lines from scratch, managing all components: people, operations, vendors, technology, and cross-functional coordination.
• Think strategically about enterprise-wide impact while also solving granular workflow problems on the front line.
• Demonstrate deep experience in population health outcomes, care quality measurement, and interdisciplinary collaboration across clinical and non-clinical functions.

🎯 Requirements

• Board-certified physician (emergency medicine, internal medicine, or family medicine preferred) with 5+ years in clinical practice, willing to continue practicing in a virtual care setting.
• Experience leading and managing clinicians, including advanced practice providers.
• Experience building or scaling a clinical program or service line, not just operating within one.
• Comfort working with engineering, product, and data teams to influence technology-driven care delivery.
• Experience designing interventions to improve population health outcomes and measuring care quality.
• Experience working in interdisciplinary teams across clinical and non-clinical functions.
