- • Provide direct telemedicine care to elderly patients with complex chronic cardiopulmonary conditions, including congestive heart failure and COPD, serving as the primary provider of record for a substantial active patient panel.
- • Conduct comprehensive telemedicine visits, including initial history and physical assessments and ongoing follow-up care, ensuring continuity across the entire care spectrum from post-acute transition to advanced care planning and end-of-life dignity.
- • Assess, diagnose, and manage adult outpatients with multiple chronic medical conditions, focusing on cardiopulmonary diseases while considering psychosocial, economic, and environmental barriers to health.
- • Collaborate daily with supervising nurse practitioners, physicians, and medical directors to coordinate patient care, optimize treatment plans, and ensure clinical safety and quality.
- • Advocate for patients facing major social stressors by activating community resources to improve access to medications, transportation, nutritious food, and safe housing, directly addressing social determinants of health.
- • Serve as the consistent, trusted point of contact for patients and families across all stages of illness, bridging gaps between primary care, specialists, home health, and hospice services.
- • Engage with interdisciplinary teams of engineers, designers, and clinical staff to identify inefficiencies in care delivery and co-develop solutions that improve patient outcomes and system-wide workflows.
- • Participate in continuous quality improvement initiatives by reviewing patient cases, proposing care protocol enhancements, and testing new clinical or operational models based on real-world feedback.
- • Educate and support referring providers and healthcare practices in the community about Ansible Health’s program, building trust and expanding referral networks through direct engagement and clear communication.
- • Maintain meticulous clinical documentation and ensure full compliance with all regulatory and certification requirements for telehealth practice and chronic disease management.
- • Manage on-call responsibilities on an alternating weekly basis, providing timely clinical guidance to patients outside regular hours and coordinating urgent interventions as needed.
- • Demonstrate intellectual curiosity by independently researching unfamiliar medical concepts, creating self-study tools such as flashcards, and actively seeking feedback to deepen clinical knowledge and decision-making.
- • Embrace a non-traditional, cross-functional role that blends advanced clinical practice, case management, social advocacy, patient education, and program development—rejecting siloed care models in favor of holistic, patient-centered care.
- • Contribute to a culture of innovation by proposing and piloting new ideas for improving patient care, including technological, logistical, or behavioral interventions that enhance accessibility and outcomes.
- • Work within a fast-paced, evolving startup environment that prioritizes clinical excellence over metrics, values radical empathy, and expects clinicians to challenge conventional healthcare norms.
- • Maintain active licensure and certification, including completion of required continuing nursing education, to ensure adherence to professional standards and regulatory compliance.
🎯 Requirements
- • Completion of an accredited graduate-level Nurse Practitioner program with a Master’s degree
- • Current NP licensure in Virginia and Pennsylvania
- • Active national certification through the American Academy of Nurse Practitioners Certification Board (AANPCB)
- • 5+ years of experience caring for adult patients in acute and chronic settings, with strong background in Primary Care, Internal Medicine, Geriatrics, or Urgent Care
- • 1+ year of direct experience managing adult outpatients with chronic cardiopulmonary conditions (e.g., COPD, congestive heart failure, coronary artery disease)
- • Reliable high-speed internet connection (minimum 25 Mbps)
