📋 Description

• • The Nurse Practitioner/Physician Assistant will be part of the Care Anywhere (CAW) program in Riverside/San Bernardino, providing in-home medical and social support to seniors and chronically ill patients to prevent unnecessary hospitalizations and improve health outcomes.
• • Day-to-day responsibilities include conducting in-home assessments (up to 5 per day) 3.5 days a week, working remotely 1.5 days a week, identifying diagnoses for care management, coordinating with PCPs and specialists, leading clinical teams including nurses and social workers, and developing care plans with the Regional Medical Officer.
