Patient Engagement Associate

📋 Description

• • The Patient Engagement Associate role is critical to Imagine Pediatrics’ mission of reimagining care for children with special health care needs by serving as the first point of contact for prospective families, guiding them through enrollment and technology setup to ensure timely access to virtual-first, in-home medical, behavioral, and social care.
• • Day to day, the associate will conduct telephonic outreach to parents, family members, and caregivers; utilize engagement strategies to walk families through downloading the Imagine app and scheduling enrollment visits; effectively communicate services and overcome objections; represent Imagine Pediatrics with compassion and professionalism; coordinate with the Local Outreach Team to support both non-engaged and engaged patients; leverage reporting and dashboards to prioritize outreach efforts; and consistently meet or exceed monthly engagement targets.
• • Imagine Pediatrics is a tech-enabled, pediatrician-led medical group delivering 24/7 virtual-first and in-home care to children with medical complexity, working alongside families, providers, and health plans to break down barriers to quality care without replacing existing care teams, but enhancing them with an extra layer of support grounded in compassion, creativity, and commitment.
