📋 Description
- • Provide compassionate psychosocial support to hospice patients and families in Lancaster/York County, PA, conducting assessments within 5 days of admission to identify emotional, social, financial, and environmental needs and develop appropriate interventions.
- • Participate in interdisciplinary group (IDG) meetings, contribute to care plan development, monitor patient and family responses to treatment, and maintain accurate clinical records including evaluations, care plans, and daily notes.
- • Join Ennoble Care, a mobile healthcare provider delivering primary, palliative, and hospice services across multiple states, committed to honoring patients through home-based care and the motto “To Care is an Honor.”
- • Develop professional expertise in hospice social work through ongoing education, workshops, conferences, and organizational engagement while supporting care coordination and discharge planning as needed.
