- • Provide inpatient hospital-level care to eligible patients in their homes through the Hospital at Home program at NewYork-Presbyterian Hospital.
- • Conduct daily in-home visits and virtual visits as part of a multidisciplinary care team delivering acute care outside a traditional hospital setting.
- • Utilize 24/7 remote patient monitoring technology to track patient vitals, respond to alerts, and adjust care plans in real time.
- • Deliver care aligned with NYP’s best-in-class standards for quality, service, cost efficiency, and innovation in the home environment.
- • Adhere to shift schedules of 8:00 AM to 8:30 PM, with a requirement of 54 shifts per month, including weekends and holidays.
- • Collaborate with physicians, case managers, and clinical support staff to ensure seamless continuity of care between home and hospital systems.
- • Perform comprehensive patient assessments, administer medications, manage IV therapies, and monitor for clinical deterioration during home visits.
- • Educate patients and caregivers on disease management, medication regimens, and warning signs requiring immediate clinical intervention.
- • Document all patient interactions, interventions, and outcomes accurately in electronic health records in compliance with NYP protocols.
- • Maintain strict adherence to infection control, safety, and privacy standards during home visits.
- • Respond to urgent patient needs and coordinate emergency transport when clinically necessary, ensuring timely escalation to hospital resources.
- • Participate in ongoing training and quality improvement initiatives to support the growth and safety of the Hospital at Home program.
- • Work exclusively at the Weill Cornell campus, 60th Street site, with no remote work permitted; all employees must live within a commutable distance to the location.
- • Contribute to reducing hospital readmission rates and alleviating inpatient capacity constraints through high-quality home-based care delivery.
- • Support NYP’s mission to become the largest, safest, and most innovative Hospital at Home program in the United States.
🎯 Requirements
- • At least two years of RN experience in Med/Surg or SDU
- • Bachelor’s Degree in Nursing
- • Current New York State RN license
- • BCLS certification through AHA
