📋 Description

• This Physical Therapist role in home health is essential for delivering personalized, evidence-based care to patients in their homes across Detroit Lakes and Fosston, MN, supporting recovery and functional independence under Essentia Health’s clinical standards.
• Day to day, the therapist evaluates patient needs, develops individualized treatment plans, implements therapy interventions, educates patients and caregivers, manages documentation and billing, coordinates with physicians and care teams, and supervises support staff while maintaining compliance with organizational policies and professional ethics.
• The role is part of Essentia Health’s Homecare department at St. Marys Hospital in Detroit Lakes, a mission-driven healthcare organization committed to holistic patient care, clinical excellence, and community well-being across rural Minnesota.
• In this role, the therapist can deepen clinical expertise in home-based rehabilitation, grow leadership skills through mentoring and committee participation, pursue advanced certifications, and contribute to improved patient outcomes while enjoying a stable weekday schedule with no weekend, holiday, or on-call obligations.

🎯 Requirements

• Bachelor’s, Master’s, or Doctorate degree from an accredited physical therapy program
• Current physical therapy license in the state of Minnesota
• BLS certification or ability to obtain within one month of hire
• Ability to meet Essentia Health’s driving requirements for home visits
• Strong communication, professionalism, and collaboration skills with multidisciplinary teams
• Commitment to adhering to Essentia Health policies, code of conduct, and professional ethics
