- • Own and manage a full caseload as a primary case manager, accountable for patient therapy plans, progression, and long-term outcomes—not just individual visits.
- • Conduct comprehensive in-home assessments to evaluate mobility, balance, strength, functional capacity, and home safety risks.
- • Develop and execute individualized plans of care focused on gait/balance training, transfers, strengthening, functional mobility, fall prevention, and assistive device recommendations.
- • Set measurable, objective goals for each patient and adjust treatment plans promptly when progress stalls or conditions change.
- • Coordinate multidisciplinary care by communicating changes in patient status to RNs, MDs, and other care team members, ensuring alignment on orders and treatment goals.
- • Supervise and delegate tasks to Physical Therapist Assistants (PTAs) where appropriate, maintaining clinical oversight and accountability for outcomes.
- • Coach patients and their caregivers on practical, sustainable home exercise programs that promote adherence and independence.
- • Document all patient interactions, assessments, and progress promptly and thoroughly in Homecare Homebase (HCHB), ensuring defensible, consistent, and compliant records.
- • Travel daily within an assigned territory to provide skilled physical therapy services in patients’ homes.
- • Transport and use basic therapy equipment safely and efficiently during home visits.
- • Maintain strict adherence to treatment frequencies, orders, and patient goals with reliable follow-through.
- • Prioritize patient safety through evidence-based mobility work, including fall risk assessment, environmental modification education, and safe transfer techniques.
- • Operate independently in the field with strong clinical judgment, recognizing when to escalate changes in condition to supervising clinicians.
- • Leverage company-developed technology tools to reduce documentation burden and improve scheduling efficiency, including the upcoming scheduling product designed to minimize last-minute changes.
- • Work a full-time schedule Monday–Friday, with occasional weekend visits as needed to meet patient care demands.
- • Contribute to a mission-driven model focused on expanding access to high-acuity, patient-centered home health care across Texas.
- • Collaborate with an experienced clinical and operational support team that enables focus on patient care rather than administrative logistics.
- • Earn compensation based on a productivity point system: $75.00 per point, with an expected target of 30 points per week, leading to an annual income range of $100,000–$130,000.
🎯 Requirements
- • Active Texas PT license in good standing
- • Strong clinical judgment and comfort working independently in the field
- • Comfortable escalating changes in patient condition to RN/MD and documenting rationale clearly
- • Reliable follow-through on orders, treatment frequencies, and patient goals
- • Current driver’s license and ability to travel daily within assigned territory
- • Ability to carry and transport basic therapy equipment
