- • As a Practice Transformation Manager at Vytalize Health Inc., you will serve as the primary point of contact between the company and assigned provider practices, leading the on-the-ground implementation of value-based care programs to improve patient outcomes and advance VBC contract performance.
- • Day to day, you will lead the implementation of value-based care models, optimize clinical and administrative workflows, implement and optimize technology solutions like EHRs and care management tools, ensure regulatory compliance, develop population health programs, facilitate care coordination, assess and manage risk, develop performance measurement systems, and build strong relationships with practice administrators, providers, and internal teams.
- • Vytalize Health is a rapidly growing organization focused on building a market leader in value-based healthcare, working with physician practices to drive population health management, clinical workflow improvements, and quality performance initiatives.
- • In this role, you will develop expertise in value-based care models, population health management, care coordination, and performance measurement while driving measurable improvements in practice performance and patient outcomes, positioning yourself as a leader in healthcare transformation.
🎯 Requirements
- • Bachelor's degree in Healthcare Administration, Public Health, Nursing, or a related field
- • 3+ years of experience in practice transformation, care coordination, population health, or a related healthcare operations role
- • Direct experience working with physician practices or provider organizations in a value-based care environment
- • Strong understanding of value-based care models and primary care practice operations
- • Ability to manage multiple practice relationships simultaneously while delivering measurable results
- • Effective communicator with strong presentation, follow-up, and stakeholder management skills
