  • • Embark on a transformative journey in healthcare as a Registered Nurse (RN) for Avera Health's innovative Hospital at Home program, based at Avera McKennan Hospital in Sioux Falls, SD.
  • • This pioneering role places you at the forefront of redefining inpatient care, delivering acute-level nursing services directly to patients within the comfort and familiarity of their own homes.
  • • You will be an integral part of a dynamic, multidisciplinary team, collaborating closely with hospitalists, advanced practice providers (APPs), therapy specialists, and pharmacists to provide comprehensive, evidence-based clinical care that mirrors inpatient standards.
  • • The Hospital at Home model is designed for patients with low to moderate acuity needs, offering them a high-quality alternative to traditional hospital stays, typically for a duration of 1-2 days.
  • • Your primary responsibility will be to provide acute-level nursing care, ensuring patient safety, comfort, and optimal recovery in the home environment.
  • • This involves conducting thorough patient assessments, developing individualized nursing care plans, implementing interventions, coordinating care with various healthcare providers, and educating patients and their families about their condition, medications, and self-care strategies.
  • • You will be instrumental in developing nursing care plans by obtaining detailed medical histories, performing physical and psychological examinations, identifying influencing social factors, and conferring with patients and their families to establish clear nursing objectives and define necessary interventions, schedules, and timetables.
  • • A key aspect of your role will be patient and family education, empowering them with the knowledge to understand their condition, manage medications effectively, and master self-care skills, while also answering their questions and providing essential resources.
  • • You will continuously evaluate patient care, adhering to therapeutic standards, measuring health outcomes against established standards, reviewing patient records, identifying changing conditions and needs, and re-evaluating care objectives to modify the nursing care plan as required.
  • • Maintaining a safe, secure, and healthy patient care environment is paramount. This includes strict adherence to infection-control policies and protocols, safe medication administration and storage procedures, and controlled substance regulations, while also complying with organizational standards, policies, and legal regulations, and proactively alerting others to potential concerns.
  • • Meticulous documentation of patient care services through charting in patient and department records is essential for continuity of care and regulatory compliance.
  • • You will also be responsible for ensuring patient care equipment is operating correctly by following instructions, troubleshooting breakdowns, maintaining adequate supplies, and performing preventive maintenance.
  • • Proactive management of patient care supplies, including checking stock, anticipating needs, placing and expediting orders, verifying receipt, and stocking items, will contribute to efficient operations.
  • • You will maintain competence in all nursing capabilities as outlined by leadership, ensuring you are up-to-date with best practices and advancements in home-based acute care.
  • • The role involves functioning as an interventionalist in the home, utilizing your clinical expertise and problem-solving skills to make informed recommendations to physicians based on comprehensive physical, psychosocial, emotional, spiritual, and bereavement patient assessments.
  • • You will implement the nursing process through highly proficient assessment skills during Start of Care visits, post-hospital evaluations, patient re-certifications, discharges, and supervisory visits.
  • • Partnering with physicians and the interdisciplinary team to develop a collaborative plan of care with the patient is a core function.
  • • Making recommendations for the efficient and effective utilization of resources to achieve positive outcomes by implementing clinical protocols during the admission visit, tailored to the patient’s diagnosis, is crucial.
  • • You will initiate and respond to requests for changes in plans of care, determining the appropriateness of the change and whether it constitutes a Significant Change in Condition requiring an OASIS assessment.
  • • Demonstrating the ability to influence patient and caregiver behavior to follow recommended alternatives to care, including providing thorough instruction and disease management education as ordered by the physician, is a vital skill.
  • • Educating patients, families, and caregivers about their disease process, available community resources, and recommended self-care practices will empower them and improve outcomes.
  • • This position offers a unique opportunity to be part of a high-satisfaction care model, where patients frequently express their positive experiences and preference for this type of care.
  • • You will work within a flexible schedule, typically covering day/evening shifts with weekend and holiday rotations, and minimal call time, allowing for a better work-life balance.
  • • The pay range for this position is $31.00 - $46.25 per hour, dependent upon experience, with potential eligibility for a $15,000 sign-on bonus.
