Remote Bilingual Social Worker – Case Management (Special Needs Plan, SNP)

📋 Description

• • As a Remote Bilingual Social Worker in Case Management for Alignment Health’s Special Needs Plan (SNP), you will play a vital role in improving the lives of seniors and individuals with complex health conditions by addressing their physical, mental, and social needs through compassionate, whole-person care.
• • Day to day, you will conduct virtual, telephonic, and in-home assessments; create individualized care plans; provide counseling and care navigation; coordinate Medicaid benefits; promote advance care planning; document interventions accurately; partner with RN Case Managers and the Interdisciplinary Care Team; act as a liaison between members, families, providers, and community agencies; and support members during transitions such as hospital discharges and hospice referrals.
