- • This is a fully remote, phone-based RN Case Manager role focused on supporting medically complex and chronically ill seniors in Alignment Health’s Special Needs Plan (SNP), where you will help members navigate care journeys, close gaps in care, and overcome barriers to better health from a HIPAA-compliant home workspace.
- • Day to day, you will provide telephonic case management, conduct comprehensive health assessments, create individualized care plans, coordinate with physicians and specialists, educate members and caregivers on disease management, monitor progress, advocate for timely interventions, and resolve service or access issues impacting care quality.

🎯 Requirements
- • Active, unrestricted RN license in California (Non-Compact)
- • Minimum 2 years of clinical nursing experience
- • At least 1 year of case management experience
- • Proficiency with Microsoft Office (Word, Excel, Outlook)
- • Bilingual in Spanish (strongly preferred; Vietnamese also noted)
- • Willingness to obtain RN licensure in Nevada, Arizona, North Carolina, and Texas (company reimburses costs)
