• • The Remote Outreach Concierge Navigator (Bilingual in Spanish) plays a vital role in ensuring seniors and chronically ill members receive coordinated, compassionate care by serving as their dedicated liaison throughout the healthcare journey. This role is essential to Alignment Health’s mission of putting seniors first by breaking down barriers to care access and improving health outcomes through proactive outreach and support.
• • Day to day, the Navigator conducts welcome calls, onboards new members, schedules appointments, follows up on cases, resolves eligibility and benefits inquiries, coordinates with PCPs and specialists, assists with referrals and authorizations, obtains medical records, and engages in proactive service recovery and on-site member activities as needed. They maintain deep market-specific knowledge to tailor support and ensure members never feel alone in their healthcare experience.
