📋 Description

• The RN Case Manager works with the clinical department and acts as a liaison with physician practices, advocating for personalized treatment options that address a patient’s unique care needs within a value-based care model.
• The role involves using assessment and communication skills to engage with patients, deliver patient-centered care, provide evidence-based education, and coordinate care across the continuum while maintaining confidentiality and guiding patients through the healthcare system.

• The RN Case Manager can learn and achieve expertise in care coordination, transitions of care, chronic condition management, and telehealth-based patient advocacy while contributing to improved health outcomes, lower costs, and enhanced patient experience in a forward-thinking, entrepreneurial environment.

🎯 Requirements

• Bachelor’s degree in Nursing preferred, or Associate’s degree in Nursing with relevant experience
• 5 years’ experience as an RN or RN Case Manager providing complex care management
• Minimum of 3 years’ experience in Med Surg or Home Health
• Unencumbered RN license, compact nursing license, or compact nursing license obtained within 6 months of hire
• Accredited Case Manager (ACM) preferred
• Comfortable providing care management using telehealth capabilities
