- • As a RN Telehealth Administrator on Call, you will be at the forefront of delivering exceptional remote healthcare support, playing a pivotal role in the success of our Personalized Health Partners (PHP) Telehealth provider team. Your passion for providing high-touch, person-centered care will directly impact the well-being of seniors and residents across various healthcare settings, including skilled nursing facilities, assisted living, and independent living communities.
- • This dynamic role involves a blend of administrative, clinical triage, and care coordination responsibilities, ensuring that every patient receives the right care at the right time. You will be a critical liaison, collaborating closely with an interdisciplinary team to enhance patient wellness, prevent illness, and improve clinical outcomes, all while maintaining a strong focus on customer experience and satisfaction.
- • A key aspect of your role will be to directly support the PHP Telehealth provider group, acting as a vital resource for their administrative and clinical needs. Furthermore, you will be responsible for answering and triaging calls from other telehealth use cases outside of the traditional Skilled Nursing and Assisted Living environments, broadening the scope of your impact.
- • Your primary duties will encompass coordinating with telehealth providers to ensure seamless delivery of telemedicine services. This includes facilitating communication with onsite nursing teams to guarantee that all clinical needs are met efficiently and effectively.
- • You will be entrusted with overseeing the provider queue within the 'Never Alone' system, ensuring that patient requests are managed promptly and professionally. A significant part of your responsibility will also involve providing technical support to the telehealth provider team, troubleshooting any issues that may arise to maintain operational continuity.
- • A crucial element of the role involves diligent monitoring of the Electronic Medical Record (EMR) system. You will be responsible for triaging routine clinical lab requests, imaging results, and other notifications that require attention outside of standard business hours, ensuring no patient concern goes unaddressed.
- • Maintaining accurate and timely documentation of all patient encounters within the EMR system is paramount. You will also be responsible for recording pertinent information directly into the CRM system, contributing to a comprehensive patient record.
- • When patients require acute care from an in-house provider the following day, you will be the point of contact to connect them with Central Scheduling, ensuring a smooth transition of care.
- • Beyond the SNF use case, you will expertly answer and triage calls from various 'Never Alone' applications, demonstrating versatility and a broad understanding of our telehealth services.
- • Providing essential health education to patients and facility staff will be a core component of your role, empowering individuals with the knowledge to manage their health effectively.
- • You will consistently strive to deliver an excellent customer experience, fostering high levels of customer satisfaction and retention through your dedicated service.
- • Upholding ethical practices and adhering strictly to the Scope and Standards of Practice of your profession and Board Certification is non-negotiable.
- • You will ensure compliance with all state and federal regulations, guidelines, and laws governing healthcare delivery.
- • In addition to these primary duties, you will actively collaborate with the telehealth provider group, contributing to a cohesive and supportive team environment.
- • Participation in monthly staff meetings is expected, providing an opportunity for shared learning, feedback, and strategic planning.
- • During periods of downtime, you may be assigned additional project-related work.
